Chapter Relief Request Form

The National PCI Chapter Relief Request form must be completed no later than February 14, 2015 by 11:59 PM EST.

Applications will be reviewed, by a committee, in the order they are received. Funds are limited, submitting an application does not guarantee funds will be granted.

An itemized expense budget needs to be attached to page three of this online form. You may also attach any purchase receipts (flight receipts, bus deposits, materials, etc.).

Only one submission per chapter.
